.--Insurance/Surgical Coordination


INSURANCE COVERAGE

  1. Dr. Joseph Capella does not participate with any insurance, including Medicare.
  2. You will need to verify “out-of-network” coverage on your policy in order to be eligible for any reimbursement.
  3. Procedures most commonly considered by insurance companies for reimbursement:

Body Lift, Panniculectomy, Abdominoplasty, Breast Reduction.

COORDINATING YOUR SURGERY WITH INSURANCE COVERAGE:
Your deposit is required, along with medical documentation in order to begin the pre-certification process.

  • The deposit is required two (2) months prior to surgery (payable by check, credit card, money order)
  • Medical documentation must be provided by the patient from their physician(s).
    • Medical documentation requirements for abdominal procedures:
      A minimum six (6) month course of treatment without resolution of Intertriginous Dermatitis (rash, sores, irritation between skin folds) AND abdominal skin (pannis) that is large and long enough to impede normal ambulation/function.
    • Medical documentation for breast reduction:
      A minimum six (6) month course of treatment without resolution of Intertriginous Dermatitis (rash, sores, irritation between skin folds), treatment by an orthopedic/chiropractic physician, physical therapy or other treatment for back and/or shoulder issues relating to the weight strain from the breasts.
    • A “Letter of Medical Necessity” is required from your physician explaining why it is medically necessary for you to have the surgery, including chart notes and documentation of treatment for Intertriginous Dermatitis.

Once the deposit and all medical documentation have been received, the pre-authorization will be initiated.  You will be kept informed of the process.

 

INSURANCE APPROVAL IS NOT A GUARANTEE OF PAYMENT
