Fees
Dr. Capella charges $100.00 for an initial consultation. This fee will be deducted from the cost of surgery performed by Dr. Capella.

Deposit
A deposit is required to reserve a place on the surgery schedule once you have decided upon surgery. The deposit will go towards the total balance for the procedure.

Payment Policy
The balance of the surgical fee is due in full two to three weeks prior to surgery. Your fee will be the figure provided in writing at the time of your consultation, less the amount of the deposit. We accept only the following forms of payment:
